An AI receptionist for HIPAA-covered practices in Illinois
Illinois adds three things to HIPAA: every party must consent to a recording, voiceprints are biometric data under BIPA, and mental health records have their own stricter law. Your virtual receptionist announces the recording, never identifies callers by their voice and keeps clinical details with your team.

| Rule | What it says | How your receptionist handles it |
|---|---|---|
| Recording consent (720 ILCS 5/14-2) | Every party to a private conversation must consent to a recording. | Its first sentence says it’s an AI and that the call is recorded, before the caller shares anything. |
| Biometric Information Privacy Act (740 ILCS 14) | A voiceprint is a biometric identifier. Collecting one needs written notice and a written release first, and people can sue over violations. | It identifies callers by what they say, such as a name and date of birth, never by their voice. It doesn’t use voiceprints. |
| Mental Health and Developmental Disabilities Confidentiality Act (740 ILCS 110) | Records and communications about mental health services get stronger protection than HIPAA gives, with narrower rules for sharing them. | For mental health practices, the call flow asks the least: a name, a callback number and whether it’s urgent. Emails never include details. |
General information, not legal advice. Your compliance advisor should confirm how these rules apply to your practice.
Why voice data matters in Illinois
BIPA lets people sue over biometric data collected without written consent, and recent lawsuits claim that AI on customer-service lines made voiceprints from callers’ voices.
A recording of a call isn’t a voiceprint. A voiceprint is a measurement of someone’s voice used to recognize them. Your virtual receptionist doesn’t recognize callers by their voice: it asks who they are, the way your front desk does, and checks a name and date of birth where your rules call for it.
Recordings are kept 7, 14 or 30 days, as you choose in Settings, and the transcript and summary follow the same schedule. Only your team can open them in the portal.
If someone is in crisis
A caller who says they may harm themselves or someone else is told to call 911 now, and that in the US and Canada they can also call or text 988, the suicide and crisis line. That rule is in every agent and can’t be turned off. Your rules can also alert your on-call clinician. The receptionist never counsels.
A first call to a therapy practice
- AI receptionistThanks for calling Example Counseling Group. I’m the practice’s AI virtual receptionist. This call is recorded and transcribed so we can handle your request. How can I help?
- CallerI’d like to start seeing someone. I’ve been having a hard time.
- AI receptionistI’m glad you called. I can book a first session for you. Is this something that can’t wait, or are you safe right now?
- CallerI’m safe. Evenings are best for me.
- AI receptionistThank you. Dr. Kim has Tuesday at 6. Would that work? I’ll just need your name and the best number to reach you. If things get harder before then, you can call or text 988 at any time.
On every call in Illinois
- Its first sentence says it’s an AI and that the call is recorded
- It asks only for what the call needs: name, date of birth, a callback number, the reason in a few words
- Clinical questions go to your team, never answered by the AI
- It’s set up to tell anyone describing an emergency to call 911
- Call emails only say a call came in; the details stay in the portal
- Recordings are kept 7, 14 or 30 days, as you choose; the transcript and summary follow the same schedule
- Call data is stored in the United States
- A BAA accepted in the portal before the agent handles health information, and our own with every company that handles patient information
What stays with your practice
An AI receptionist doesn’t move your duties to us. You still decide who on your team sees calls, train your staff, answer patients’ requests and decide what to keep: calls stay in the portal for the retention period you choose (7, 14 or 30 days), so download any call a law or your own policy requires you to keep longer. Your receptionist isn’t an emergency service. Like any AI, it can mishear or misunderstand, so keep your own emergency arrangements.
What your missed calls are worth
Put in your own numbers.
Revenue those missed calls could be worth each month
$15,588
An estimate from the numbers above, not a guarantee. Amounts in US dollars. A month is counted as 4.33 weeks; cost is billed by the second. Starting values are example numbers chosen by 365vox for a therapy practice; replace them with your own.
Illustrative example
A group therapy practice in Chicago with eight clinicians
Clinicians are in sessions all day, and the one coordinator can’t answer every call. People reaching out for the first time often don’t leave a message.
- The virtual receptionist answers during sessions, evenings and weekends
- First sessions are booked with the right clinician by the practice’s rules
- Only a name, a number and urgency are asked; nothing about why
- Callers in crisis are given 988 and 911, and the on-call clinician is alerted
The numbers, worked out
- Calls answered that used to be missed
- 43 a month
- New clients at 40%
- 17
- First six sessions at $900 each
- $15,588
Illustrative example, not a 365vox customer result. Assumptions: 10 missed calls a week (4.33 weeks a month), 40% become new clients, $900 for their first six sessions, 3 minutes a call at $0.75. Change any of them in the calculator above.
Comparing virtual receptionist services?
